Not every narrowing on an angiogram warrants a stent, and not every functionally significant lesion is visible to the naked eye. Coronary physiology uses pressure-wire measurements inside the artery to determine whether a narrowing genuinely limits blood flow to the heart muscle. Dr Nijjer worked in the team which invented the iFR technique, now used in tens of thousands of patients worldwide and embedded in European and American cardiology guidelines.

Intravascular ultrasound (IVUS) and optical coherence tomography (OCT) place a miniature imaging probe inside the coronary artery, providing high-resolution cross-sectional images far beyond what standard angiography can reveal.
