Mapping your heart's blood supply with precision
Coronary angiography is the gold standard for diagnosing blockages and narrowings in the coronary arteries. Using either a catheter passed through the wrist or a high-resolution CT scanner, Dr Nijjer creates a precise roadmap of the heart's circulation — guiding every subsequent treatment decision with objective, anatomical data.
Restoring blood flow through blocked coronary arteries
Percutaneous coronary intervention (PCI) opens narrowed or blocked coronary arteries without open-heart surgery. Dr Nijjer is a high-volume operator with particular expertise in complex multi-vessel disease, bifurcation lesions, and left main stem intervention — among the most technically demanding procedures in cardiology — with outcomes guided throughout by physiology and intravascular imaging.
The science behind every stenting decision
Not every narrowing on an angiogram warrants a stent, and not every functionally significant lesion is visible to the naked eye. Coronary physiology uses pressure-wire measurements inside the artery to determine whether a narrowing genuinely limits blood flow to the heart muscle. Dr Nijjer worked in the team which invented the iFR technique, now used in tens of thousands of patients worldwide and embedded in European and American cardiology guidelines.
Seeing inside the coronary artery in real time
Intravascular ultrasound (IVUS) and optical coherence tomography (OCT) place a miniature imaging probe inside the coronary artery, providing high-resolution cross-sectional images far beyond what standard angiography can reveal. Dr Nijjer uses these techniques to characterise plaque composition, size vessels precisely, guide stent placement, and confirm optimal results — substantially reducing the risk of stent failure.
Unlocking severely calcified coronary arteries
Heavy calcium deposits make coronary arteries rigid and resistant to conventional stenting, leading to incomplete expansion and a higher risk of failure. Dr Nijjer specialises in the full range of calcium modification techniques — shockwave intravascular lithotripsy (IVL), rotational atherectomy, orbital atherectomy, and excimer laser. These complex procedures are often the only route to safe and durable revascularisation in high-risk patients.
Restoring and regulating the heart's natural rhythm
A pacemaker is a small device implanted just beneath the collarbone that continuously monitors the heart's electrical activity and delivers precisely timed impulses when the heart beats too slowly or erratically. Dr Nijjer performs pacemaker implantation as a planned procedure with comprehensive pre-operative assessment and long-term device monitoring and follow-up under his direct care.
